Description
This series contains inpatient and outpatient files of Hillcrest Hospital, previously Northfield Mental Hospital.
Records contained on the files may include patient notes, correspondence with the referring doctor, statement of particulars, clinical summaries, psychiatric case history, blood results and ward notes.
The majority of these files relate to patients treated between the 1960s and the 1980s. There are however a number of files which document treatment before this date - these files generally contain loose leaf sheets removed from 'Case books' and placed in a file at a later date. These sheets from case books often include photographs.
Some of the files have date ranges which extend beyond the 1980s. In these cases, the later information does not relate to treatment, but is generally correspondence relating to access to the file by the patient, family members or medical practitioners.
Many of these files have labels on the cover which record the patients personal and admission details, as well as an admission number. These numbers have been listed in consignment listing along with the prefix for the relevant hospital (includes NMH - Northfield Mental Hospital, H - Hillcrest, PMH - Parkside Mental Hospital). Some of the later files have UR [Unit Record] numbers. UR numbers were introduced at Hillcrest in the early 1980s as a manual process that was computerised in 1984/85. In 1992/93 Hillcrest patients were also allocated an additional Glenside Hospital UR number. When Hillcrest Hospital closed in 1994, patient records were sent to Glenside for storage and were maintained in alphabetical order.
Patients were often moved between institutions in the courses of their treatment, and consequently these patient files may contain documentation relating to various hospitals.
Other information included in the listing of these files is the date of birth [DOB] and other names (aliases, alternate spellings).
