Notice of death forms - Northfield Mental Hospital (GRS/16022)

This series contains 'Notices of Death' form books (Form No. 29 from Northfield Mental Hospital.

The forms record: ward, patient's name, age, date and hour of death, and the name of Attendant or Nurse present at death. Each form is signed by the Attendant or Nurse in Charge of the ward. The forms were entered in triplicate, but only one completed form was left in the book. The other forms presumably went to the Inspector General of Hospitals/Hospitals Department or the Superintendent of Mental Hospitals, and the undertaker who collected the patient's body.

Deaths at Northfield Mental Hospital were also recorded in a register (GRS 15234) and on Medical Certificates for the Cause of Death (GRS 16001).

Each book contains forms for just one ward. Only three books are included in this series and the whereabouts of other books are unknown as at July 2015. Books in this series are: Block 6 (1952 - 1957), Ward 1A, B and C (1967 - 1970) and Ward 12B (1969 - 1971). Entries are arranged chronologically.
