Description
This series contains a book of case notes from Hillcrest Hospital, possibly relating to patients in Litchfield House.
Patients recorded in this book were generally admitted under Section 32 of the "Mental Health Act", a section relating to admissions to receiving houses or wards for assessment of their mental condition.
The notes were compiled by psychiatrists at the Hospital and it appears that these were written soon after an interview with a patient, possibly to be written up or typed for their case file.
Although one particular psychiatrist's name is recorded on the front cover, it appears that the patients included in the book were not exclusively under this psychiatrist's care during their time at Hillcrest. Doctors' names are usually recorded before the patient's case note and it appears as though these doctors were supervising psychiatrists. Handwriting changes throughout the book, indicated it was recorded by several different people.
Information varies, but generally includes patient's name, age (sometimes date of birth), marital status, occupation, address (usually just a town or suburb name, or state if they were from interstate), medical history, family history and notes about their personal life. There are some notes about what medications they had been prescribed, as well as notes about their physical appearances. The notes usually include the patient's diagnosis and some include treatment plans.
The book also contains the minutes of the 'Litchfield House Policy Meeting' from 11 September 1975.
Only the book from 1975 is included in this series. If other records of this nature were kept for other years, their whereabouts are unknown as at October 2015.
