Description
This series contains books recording certificates in respect of tranquillising drugs and/or anti-depressant drugs. The books are labelled Commonwealth of Australia, Department of Health on the cover.
The object of the certificates was to enable a patient who has been discharged from an approved hospital to have certain tranquillising and/or anti-depressant drugs prescribed for them as pharmaceutical benefits by their private doctor and dispensed by an approved chemist.
The certificates were completed in triplicate with the original forwarded to the patient's doctor, the duplicate to the Commonwealth Director of Health in the State of issue, and the triplicate retained for record purposes.
The certificates record patient name, address, prescription and dosage, medical practitioner, name of approved hospital and date of issue.
Multiple hospitals are recorded in some volumes. It appears likely that some of the volumes were issued to doctors who attended patients at more than one hospital.
