Cause of Death Medical Certificates - Julia Farr Centre (GRS/18679)

This series contains cause of death medical certificates from the Julia Farr Centre from 1984 to 1999.

The certificates were completed when a Julia Farr Centre patient passed away. They certificates were pre-printed forms in volumes, with the certificates able to be removed to be forwarded to the appropriate authorities and a stub remained in the book. The stub records: name of the deceased, age, last time they were seen alive, date and time of death, cause of death, signature of the person filling out the certificate, and date signed.

The books were kept on individual wards, although some do not have any ward name recorded on them. As different books were used in different wards, the volumes' date ranges overlap.

The books have an imposed numerical system (1 - 31), applied chronologically.
