Advice of admission and advice of discharge forms - McNally Training Centre, later South Australian Youth Training Centre (GRS/6642)

This series comprises duplicate copies of Commonwealth Department of Social Services (later Security) forms relating to boys admitted and discharged at the McNally Training Centre, later South Australian Youth Training Centre, at Magill between 1970 and 1986 (with gaps). The forms appear to relate to the calculation and payment of family allowances.

Consignment 1 contains yellow carbonless paper copies of pre-printed forms entitled: Department of Social Security 'Advice of Discharge of Child From Institution', 1983 - 1986. These Commonwealth department forms record: advice serial number, name of institution, name of child, date of birth, date of discharge, name and address of person or institution to whom child was discharged, signature of authorised officer. Copies 1 and 2 were returned to the Department of Social Security, with a copy being retained by the Department for Community Welfare.

Consignment 2 contains green carbonless paper copies of pre-printed forms entitled: Department of Social Security 'Advice of Admission of a Child to an Institution and Claim for Additional Family Allowance', 1982 - 1986. These Commonwealth department forms record: Advice serial number, name of institution, name of child, date of birth, date of admission, name and address of person last receiving family allowance for child or with whom last reside, name and address of child's mother, signature of authorised officer. Copies 1 and 2 were returned to the Department of Social Security, with a copy being retained by the Department for Community Welfare.

Consignment 5 contains 'Advice of Discharge of Child From Institution' and 'Advice of Admission of a Child to an Institution and Claim for Additional Child Endowment' from 1970 to 1972. The description of the forms is as above, although at this time the Federal department was the Department of Social Services.

Within each consignment, the forms are arranged alphabetically by surname within year.
